Affirma assisted our agency in recreating our hospice and palliative care referral board. Our Referral Department went from tracking data on a physical white board to an Excel spreadsheet, and with Affirma’s skills, we were able to go to a electronic board that can be seen live time by several different parties within our agency. I feel that our Snowline team was given full opportunity to collaborate, share our vision, and test the new dashboard prior to its launch. It has been almost a month and our team is using the board with much ease and comfort. Other viewers within the agency are able to access the patient referrals simply as well.
